Community Access

Personal Care

Our Personal Care services, compliant with NDIS standards, preserve dignity and enhance well-being. We support our participants with a calming presence, compassion, and person-centred care. We assist with daily self-care tasks including hygiene, grooming, toileting, eating, using aids, and mobility assistance. Our support extends across homes, during activities, and with family or others.

Community Nursing

Our community nursing services provide a wide range of care, including support for complex bowel care, urinary catheter management, continence assessment, enteral feeding, ventilator and tracheostomy care, medication assistance, and epilepsy and seizure support. Our nursing professionals conduct thorough assessments, develop care plans, coordinate health needs, make referrals, and offer education and training to support staff and family members.

Psychosocial Counselling

Psychosocial counselling involves a supportive relationship between a counsellor and an individual facing social, emotional, or behavioural challenges. Through assessment and personalised plans, specific well-being goals are targeted. This process focuses on understanding circumstances and providing tailored guidance for growth and resilience. Alex

Founder

Alex brings a diverse background starting with surgical nursing from 2007 to 2013, followed by earning a Master’s in Mental Health from 2016 to 2018. His transition into community mental health nursing stemmed from a desire to do more than surgical procedures. Passionate about supporting individuals often misunderstood by society, Alex recognised the unmet needs of vulnerable populations and seized the opportunity to make a meaningful impact.

Director

Decontee, with a background as an enrolled nurse and extensive experience spanning over 15 years in the disability sector and having completed a certificate in disability support services, plays a pivotal role in overseeing support staff coordination services. She brings knowledge and a deep commitment to enhancing the quality of life for individuals with disabilities.
